Forms are only sent to people who are in the group assigned to the form. If a request for a form is received from a number that is not in the group, FrontlineSMS will not send the form. Could you check that the user who requested the form is in the group? Also please check the SMS inbox on the phone - some phones fail to read the incoming sms as a form and instead save it to the normal messaging inbox.
If this does not work, please send us your logs and we will see if there is a technical issue preventing the form from being sent.
That's not the case, because the contact is actually in the group, since it receives the message of "there is a new form available". The thing is no message is sent from the modem when the request is received... If you don't have more suggestions, I can send you the logs.
