Unfortunately there's currently no provision for deleting a form from within FrontlineForms on a handset. You will have to delete the application, which will delete all the forms."

Are you plugging the phone into the same USB port each time? Using different USB ports may be the cause of this problem. Also, try plugging the phone in first, and then starting FrontlineSMS, in that order.

Let us know the type of phones, whether there has been any update to those phones from the operator, what kind of modem or device the main hub is using, all of that. :)
